It looks like the auto-adjuster for a drum brake. In cars, this allows the brake shoe to automatically move closer to the drum as the shoe wears down, so it maintains the same distance. 1865 is a little early for cars and maybe even street trolleys, I think those had hand operated brakes. But it probably serves the same purpose, to allow automatic adjustment of something... can't imagine what though.
