I use a Flirc USB with my Fire TV and then a basic Logitech 350 (refurbished to save me some $$$).
Much nicer looking and it works surprisingly well.
This guy. I've loved it. Does my Apple tv. Tv receiver and DVD player. My kid likes to grab my Apple TV remote and grind it on the tile as he crawls sound the house. So I hide it and his big sister uses this remote.

The Logitech Harmony remotes are excellent, support just about everything, and are dead simple to program from your computer.

No, but you could get an IR remote and receiver. There are fancy ones too but come at more cost.
As far as I know, you would need 3rd party software to accomplish switching monitors such as MultiMonitorTool. Assuming you have windows 8 64bit, here is the direct link.

Logitech Harmony 350 refurb $30 - Simple remote that you can program with your computer.
Send it back. The shields bluetooth sucks and pretty much only works with it's own proprietary devices which use wifi instead.
