Something like this should do the job, male ends into your amp and computer headphone jacks and female end for your headphones.

Does your computer support mic/headphones in one plug?

You might need to order a splitter such as this one and use both mic and headphones plugs: http://www.amazon.com/Monoprice-6-Inch-Stereo-Splitter-Mobile/dp/B00DIGCZ3S/ref=sr_1_10?ie=UTF8&qid=1451928617&sr=8-10&keywords=3.5mm+splitter

À splitter like this should do the trick (mind you there probably are a lot cheaper options)

Yes the headphones only have 4 pole 3.5mm connector. The combo, 4 pole, port can do the mic and audio over the one connector. Cellphones have combo ports and some laptops like my Lenovo Y700. It looks like ps4 and XBone also use it. Try the headpones on your cell and it should work with the mic and audio. You could buy a splitter to make them work. https://www.amazon.com/Monoprice-6-Inch-Stereo-Splitter-Mobile/dp/B00DIGCZ3S/ref=sr_1_3?s=aht&ie=UTF8&qid=1468834535&sr=1-3&keywords=3.5mm+splitter

EDIT: Or just plug the mic into the computer directly.

Something like this might be your best bet.
