With my GH5, I mount both a Sennheisner MKE400 and a receiver to this Audio-Technica Wireless Lav to the camera using a SmallRig cage. I use a cheap Hosa splitter to separate the two channels. Now, I can run around and get decent ambient noise via the shotgun mic, and I also can quickly add the lav for interviews. It's a fantastic run-n-gun setup that has very impressive results.

Otherwise, I just use a Tascam DR-40 and do everything externally.

I use a Smallrig cage. I like the way it goes the body and it seems very well made for the price. I use the xlr adapter on my GH5 so I picked up the cage designed to fit that.

That said, shoe extensions can be tough on your hot shoe when you load them up with recorders, mics, top handles, etc.

If I had it to do all over, I'd get a [SmallRig cage] (https://www.amazon.com/SmallRig-Panasonic-DMW-XLR1-Upgraded-Version/dp/B075R7238M//ref=as_li_ss_tl?ie=UTF8&linkCode=ll1&tag=battleforthew-20). Cages cost a little more money, but they also provide more places to mount your gear - and they're easier on your camera.
