If you have a computer place nearby or work with an occupational therapist perhaps ask them for help. There are joystick options, some ergonomic mice may fit your hand more easily. Lots of stuff with non-traditional shapes. I searched for joystick mice on amazon and came up with lots of different stuff.

If you definitely must use a mouse without a desk, consider a handheld trigger mouse. I use this style of mouse while using a treadmill.

something like this seems to me to be a better solution to your HID problem. requires one hand to operate, can easily be stashed in a pocket to free your other hand.
i remember seeing ones that use a 'joystick' rather than a ball, but i cant be bothered to dig and find them.
Try moving the receiver around to better locations. Also, if you are using rechargeable batteries, the signal is going to be weaker (rechargeable batteries are 1.2V, rather than 1.5V).

Do any of these look like they could work? If not, we can concentrate on your current mouse, but I think there's quite a few large hurdles in that effort.
There are small, wireless handheld devices with a trackball integrated. They fit in the palm of your hand and are meant to be used in office presentations and the like.
This is one example though I have no experience with this class of device let alone the linked model so I can make no recommendations. Other than saying it sounds much more convenient than a desktop model for your circumstances.
edit: I see in another reply that you have considered these already, but I would add that if you aren't using one for more than 5-10 minutes while selecting your file/stream/video the ergonomics are not likely to be a huge factor.

Könnte sich in dem Fall lohnen, mal bei drahtlos-Eingabegeräten zu schauen. Mäuse gibts da ja schon länger, mittlerweile gibts aber auch einigen anderen Kram in ordentlich verwendbar. Für den Apfel gibts dieses Magic Trackpad (und die Touchpads von denen sind bekanntermaßen echt gut). Oder sowas wie das Teil hier. Taugt zwar nicht, wenn man häufig Tippen muss, aber für Präsentationen echt geil (jedenfalls wenn man Trackballs mag Ü).
FYI a Thumb mouse is one of these.

You know, you can get a small-form OTG adapter and a Logitech M317 for under $20. It'd be a lot less irritating being wireless and tighter to the phone.
For under $30, he could go with a wireless handheld trackball.
