That's just it - I didn't change anything about my diet or meals. Instead, I just kept (and still keep) myself well hydrated. This doesn't mean I obsess about hydration; it's just a basic goal that I think of and consider each time I pee. It takes about half a second; urine is dark then drink some water. After a while, it just became a habit and seems absolutely effortless. And, I notice that I feel better in general when I drink enough to keep my pee close to clear.

I think one of the biggest problems people have is that they feel overwhelmed by the amount of nutritional advice out there. For good reason: a lot of it is contradictory, the advice constantly changes with the release of new studies, a lot of it is junk science, and it seems impossible to satisfy all the requirements we're given. I subscribe to a much simpler mindset: just eat healthy foods and you'll get the nutrition you need.

Michael Pollan has written a number of books about diet and nutrition. His background is investigative journalism so he's well-suited for analyzing the confusing information out there. And, he's a great author! Michael Pollan has summed up his research in three simple rules:

I'll add a fourth rule: eat lots of colors. Different colors typically indicate different nutrients. Eat a lot of colors throughout the day. But, forget brown and tan colors (e.g., potatoes, pasta); they're not unhealthy, they just tend not to provide too much in the way of nutrients so don't go patting yourself on the back because you ate four differently colored potatoes today.

For a quick read, grab a copy of Pollan's Food Rules. Most of the pages are half-blank so it's a quick read. When you're done with that, pick up any of his other books; they're all great.

Obviously your wife doesn't want to lose too much weight while pregnant, but that's okay. She should just eat enough calories of DASH friendly foods to maintain her weight.

You can also download an app like LoseIt! or My Fitness Pal, and start counting grams of sodium. For hypertension, you should stay under 1500 grams per day. These apps will also count calories and keep food logs, so very helpful to show the doctor.

Let's break it down this way instead:
PCOS is an inflammatory condition. This is one of the reasons, in addition to the crazy hormones that will make people think you're nuts, is why your body in general just feels icky.
Every diet is different for people because people are different! For example a lot of PCOS people do Keto. I don't because it makes me feel gross. My doctor actually suggested to do a balance of Whole30 and Paleo or Mediterranean depending on budget.
One of the big things in the U.S. is our food. Processed foods, which include cereals! are just in general really bad for you. Great to have in an emergency but that's where a little bit of meal prep and freezing is really handy. For things like rice, I actually bought myself a giant bag from Costco and I'm just slowly working my way through it. It lives under my sink and I got a rice maker from Bed Bath and Beyond for under $20 with a coupon (this is where Amazon is your friend). For now, I would start very simple. Cut out the sugar. Cut out cheese if you're a cheese hound! Move from diet pop to something like La Croix or Bubly -- I did this and was amazed at how much better I felt as I'd always been a Caffeine Free Diet Coke drinker (in addition to tea and water).
Don't. Overthink. Listen to your body and track how your body feels after you eat certain foods. Fried foods and carb heavy meals make my anxiety levels go up and I feel icky afterwards for example.
The important thing is to educate yourself and find what works best for you.

That depends on your concept of healthy. I'll give you mine.
I use a Primal diet, because it is low-carb and gluten-free, but there's a positive focus on things I can eat instead.
Gluten has been linked to some odd symptoms. It's possible it has some tie to the mystery illness. Or she could try Whole30, which is a bit drastic, but I hear that many people have found it helpful to identify unknown food allergies. Essentially you eliminate several common allergens from your food for 30 days. After that, you add them back one at a time, and see what causes problems.

This is probably the best one for a beginner, and it does include nutrition data:

Paleo Comfort Foods -- Bigger book with more recipies. Has some really good low-carb alternatives to desserts or typical comfort foods.
All of those have great pictures and reasonable instructions. Some of the ingredients are a bit odd.
I normally eat non-organic fresh foods, but lately I've been thinking more about going organic with food and all beauty products. I am not really afraid of GMOs right now, but am always reading about nutrition, health and stuff, so maybe something will convince me to switch.
I recently watched Michael Pollan's In Defense of Food on Netflix, and found it really inspiring and informative to start eating more real food, and focus less on the calories. As he mentions (or maybe this was in Fed Up, which I watched immediately after), I have the unpopular opinion that a 200 calorie candy bar is metabolized different than 200 calories of unprocessed food, and the fiber/sugar differences are incredibly important.
Diet foods like Diet Coke and Halo Top are often really appealing to me, because I do have such intense sugar cravings, but these are seeming more and more like poison.
This is a really awesome book by Pollan, illustrated by one of my favorite artists. And here are his food rules.
While the dollar menu appears to be an economical way to eat, it really isn't food (mostly processed corn products, fat, and sugar). You are not getting nutrients from the dollar menu, only calories. Add to the fact toastybeast's point about future healthcare bills, and the dollar menu isn't even a good deal anymore.
Furthermore, while healthy pre-made food is expensive (and not always that healthy), healthy food you make at home is an awesome deal. Especially if you have a good farmer's market, local vegetables are cheap (especially if you don't care about them being organic). Packaged "Food products" are usually the most expensive items in your shopping cart, which you won't be needing anymore if you are eating healthy and cooking for yourself. Also, cutting down on meat consumption (a big factor in heart health) cuts down on your grocery bill.
For example: I can make 4 juicy hamburgers and sweet-potato french fries at home for the cost of one hamburger and fries at a diner, and mine have 1/4 the fat (and taste way better).
On average, Americans spend less of our income today on food than we did in the past. Think about all the things you spend money on that are way less important than your health. I bet you could probably afford to buy healthier food if you really tried.

Basically everything that midsummernightstoker said is spot on. If you're interested in cutting down on your meat intake, the best thing I've found is to find some really solid (and tasty) recipes. The Plant Based Cookbook is super helpful because it provides you with a guide to ween yourself off meat. But also there are a lot of great recipe blogs out there, like Cookie and Kate. Eating vegetarian is a lot easier and tastier than you might think.
I've never done keto. I've always had excellent results on VLCD (very low calorie diets), which obviously give fast results, but you have to work hard to keep the weight off. These diets tend to be very low carb diets, putting you in ketosis. However, being on a VLCD is not necessary or realistic long-term for PCOS management and weight loss, which are your aims. I found a good PCOS nutrition book to be helpful when I was first diagnosed. It helped me understand what I should avoid (refined carbs and sugars, basically) or at least reduce my intake of (carbs and red meat) , in order to help my symptoms. Seafood is great, and definitely better in many respects than meat. It contains less chemicals (vaccinations etc), a lower harmful fat content, and less hormones to interfere with your own. Keeping your blood sugar levels steady and making the best food choices is achievable when you're taking a balanced approach, following, for example, a low GI regime. That way, you'll be avoiding the things that make symptoms worse, you'll reduce your risk of diabetes and insulin resistance, and you'll lose weight. I think taking note of calories is also very important.

It doesn't have to be a choice between the two. A lot of stuff is delicious and good for you. What are some of your favorite foods? It's really easy these days to find healthy versions of anything that actually tastes good.
Lots of random tips follow:
Some stuff is ok for you, just in small doses. Eat 2 pieces of pizza instead of 4, for example. When snacking, look at the serving size on the box or bag, and take out only that much.
A lot of people eat out of boredom, or because it's there. When you're gaming, instead of a bag of chips and soda, get a bag of carrots, some lite ranch, and chocolate milk.
If you can, get the junk food out of the house or somewhere that's harder to reach. It makes you think about what you're eating before you eat it.
When I started eating healthier and was trying to change my habits, I would have a "cheat day" once a week where I could eat whatever I wanted. That meant that I could tell myself "just gotta make it 4 more days", etc...
Eventually, after not eating it for a while, grease and soda don't taste as good, even a little gross. When I was in high school I joined wrestling (great way to lose weight BTW). After eating well all season, soda actually stung my mouth when I drank it. So I didn't.
I assume your parents make your dinner, but if you take over a couple days a week, making food from this book (almost everything in it is great), I'm sure they'd appreciate it, and it'll give you some great life skills for when you're out of the house.
I love the prevent and reverse heart disease cookbook. None of us have heart disease or any problems at all, really. It's just got some great recipes. They will work beautifully for a diabetic.

The 64 guidelines in that book boil down to:
DO NOT EAT COCONUT OIL!
It's very high in saturated fat. You might as well be eating butter.
Healthy nuts, like almonds and walnuts, have been shown to increase HDL, which removes plaque from the arteries.
But you should still lower your overall oil consumption (as oil is so calorie dense a few extra tablespoons a day will make you fat...and that fat will add to arterial plaque).
Fiber helps a lot to lower your cholesterol. So eat oatmeal. But don't overload it with sugar as pure sugars will damage your arteries with inflammation which will cause blood clots and more plaque to form (but fruits like blueberries are okay, actually they're beneficial).
Dr. Esselstyn (a former heart surgeon) has been the only doctor to prove his diet is capable of "dissolving arterial plaques" by using MRI scans on his patients.
Read his book if you are serious about your heart health.
Although I agree with your intent, I don't think an education in nutrition is the right approach. To me the only answer is to actively avoid all processed foods. You can't cut them out (in the US today) but you can do your damndest. As you say, eating out is an exercise in cleverly disguised (my additions) salt, fat, and sugar. Even in the most locavore, organic, vegetarian restaurant you don't know exactly what's in your dinner.
I'm not touting anyone as an expert, but Michael Pollan in Food Rules does a really nice job of creating a simple set of rules that you don't have to memorize or think about. You can eat heathy(er) without analyzing Omega-3 content, sugar content, or looking for phyto-nutrient rich super-foods.
I like the American Heart Association cookbook for all it's meat recipes because my partner needed the diet change as recommended by the doc and the recipes are versatile enough to use any meat with. Some of these are too bland for my taste. It' a hit and miss as with anything however when it hits boy does it!

This is a diet originally designed to manage high blood pressure, but it is also somewhat of a scientific consensus on what a healthy, calorie controlled diet should look like.
There are a ton of cookbooks out there, so my recommendation would be to buy one you like and follow the instructions. Try out some of the recipes and then find 5 or 6 of them you can do over and over again.

Eating real food (sounds simpler than it actually is in practice these days) and maintaining a regular fitness schedule is absolutely your best option for sustainable fitness/weight loss.
Just keep it simple and easy to remember and follow. Make it so simple that a 4th grader could convey the idea correctly to his mama or papa. Also, there's a an interesting book with some very easily digestible tidbits on food called Food Rules. It takes about 45 min to read. I recommend you do that and incorporate the relevant information.
In case you are not familiar already, check out Eating For Life. It's not really a program to follow; it's more of a handbook for balancing one's diet, and it has a bunch of example recipes and meal planning guidance. For me, it was the key to staying in top shape while no longer being a competitive athlete. The author also advocates an "off day", and I think that's a big part of making it practical. Once I got really lean and started putting on muscle, though, the off day urge went away.

Hi! It's so great you've decided to make a change! I'm about the same height and started losing weight about a month ago at 202 pounds. And I also have PCOS, I've probably had it since I was 14 or 15, but only diagnosed at 26! So far I have been able to manage the PCOS without drugs, just supplements and some massage therapy for pain relief (I also have fibromyalgia, so know what symptoms are from what is hard). But, here is what seems to be working for me right now... I joined the gym and do low intensity cardio and light strength training (all body resistance, no really heavy weights) and I joined My Fitness Pal to track calories, that has seemed to be the biggest help so far. Just by getting to the gym and moving and eating less I'm down 11 pounds since mid October, which in my mind with PCOS is decent.
I don't know what will work for you, but I've been told over and over, that once you get your weight down your hormones will balance out and the PCOS symptoms will decrease. I know it's a catch 22, PCOS makes it hard to lose weight, but losing weight is what helps PCOS. I just think every little bit helps. But you can do it! You've done the hardest part already by wanting to start.
You could take a look at The Primal Blueprint Cookbook. It has some good recipes in it and some interesting ideas. I never really thought of things like using grated cauliflower in place of rice in my meals, I haven't tried it yet but supposedly it has a similar consistency and taste. If you ever go to Mark's Daily Apple he also has some free e-books for cooking with recipes that users submit.
For eating I suggest Food Rules it's a really quick read but a good reminder on nutrition and how to make better choices.
If it doesn't have a decent amount of fiber in the carbs, I don't eat it.
I prefer the carbs to be naturally occurring, so enriched "fiber added" bread doesnt count!
So beans, fruits, veggies, whole grain breads typically fit the bill. Whereas cookies, soda, fries, fast food, etc.
Also, I really liked Michael Pollan's Food Rules. It's such a quick read and has tips like, "If it comes through your window, you shouldnt eat it!" "If you're not hungry enough to eat an apple, you're not hungry."
Great book if you just have a tendency to grab silly things at the store.

I'd highly recommend checking out this book by Michael Pollan. Each Chapters about a page long and covers one simple rules about eating healthier that anyone can follow.
Body for Life, Eating for Life, and Transformation by Bill Phillips. Transformation is his latest book. If you want just one, get that. Body for Life is pretty cheap though (especially on half.com).
I went from 240lbs with a 42" waist to 160lbs and a 32" waist back in high school. I'm on the diet again (college messed me up) and I'm down 50lbs in the last 5 months.
This reminds me of few things in Food Rules. This edible food like product would deserve to be eaten consciously instead of in front of a computer.

It's a shifting of perspective, from 'food as an intoxicant' to 'food as nourishment'.
The new perspective is the breaking of the buried seed; the weight loss is the growth of the tree.
First recognize who you are, what you are, what food is. After recognizing, let weight loss happen.
If you don't trust that weight loss will happen by itself, then you have recognized nothing at all.
