Yay! Two books I can wholeheartedly recommend keeping on the coffee table are this one and this. (NSFW). The first one in particular is actually very good. I'm sure scuttlebutt would be delighted to see that you take your health so seriously. 😋

I would recommend A Hand in the Bush: The Fine Art of Vaginal Fisting by Deborah Addington. It's a fantastic book.

Otherwise:

• Trim and file your nails! Make sure that nothing is going to catch or cut when you put your fingers inside her.
  
• Make sure your hands are clean, as well. Always scrub up before going in.
  
• Latex gloves are really good for fisting -- another layer of insurance so that you won't scratch her, easy to make sure that your hands are clean, and can be disposed of easily instead of washing gunky lube off your hand when you just want to cuddle with your gf.
  
• Speaking of lube, invest in a really good water-based lubricant. No warming or flavored lubes, no oils, no vaseline. Water-based lube ONLY. For vaginal fisting, lube is your bestest friend. You will need a lot of it, trust me. Get a bottle that either has a flip-cap, or even better, a pump on the top, cause you'll be adding lube one-handed. If there isn't goop dripping everywhere, you probably aren't using enough. Lots of lube is good.
  
• Take things slow. REALLY slow. Don't go into it thinking that you're going to get your whole hand in there on the first try. It might take 3 tries, or 30...or you might never be able to fully fist her. And that's okay. It should be more about the fantastic sensations that you can give her, not some kind of "goal" to achieve. My husband has never been able to fully get his hand in me, because he's like a foot taller and 100lbs heavier. It happens, sometimes hand size and pussy size don't line up -- but she's still going to enjoy what you can do with your fingers, so don't feel bad.
  
• Seriously, take it slow. Start with one finger, move up to two (and as you said, two fingers is all she's ever taken before), and then move on from there with care and a LOT of attention to her and her reactions. Make sure she feels comfortable getting graphic with how she tells you what she's feeling and wanting -- she is the one driving this bus, you're just following her.
  
• Don't push too far or too hard. Tearing is no fun. If the sensation is more of a warm, almost-sore-but-not-quite-pain, it's usually okay to keep going (gently!). If she starts to feel a sharp, tearing kind of pain, it's time to back off and decide whether she wants to keep trying, but more gently, or stop for the time being. Like I said, she's the one making the executive decisions, so abide by her needs.
  
• Finger positioning: Once you're ready to start putting four fingers in, in preparation for sliding in your thumb and getting your fist in there, make a duck-bill shape with your fingers, folding your hand into kind of a cone-shape. As you push forward, you will encounter resistance as the widest part of your hand meets the vaginal opening. She should be paying attention to her own body, and let you know if she feels sharp pain at any time -- if there's pain, back off and reassess. Once the widest part of your hand is inside, gently let your fingers curl as you move deeper.
  
  Dang it, I didn't notice that ArchangelleSyzygy's (excellent) link was text, not video. I read it as being video, not text. Congrats on the progress—and, more importantly, on having fun together!

Large Pearce is quite a jump in circumference from the medium: 2.25". The taper would make it easier, but I still think it's too big of an increment. 1" circumference increments are much safer.

It might be a good idea to measure the circumference of your fist. This will let you know exactly how much progress your wife still needs to make to be able to take it.
