The hole is just that perfectly sized, and the downstem grows slightly near the neck to fit 18.8 so it sort of wedges in.

These are the bits I used. I originally bought them intending to use a 12mm glass slide and grommet you get from a headshop, but the downstem I had from a broken bong just coincidentally was perfectly sized to fit without need (or room) for a grommet.
