I dont have any elbow guards, but just recently tried out two different knee guards. I'm 6' even. 255lbs.
17.750" (45.09cm) 4" below knee
21.5" (54.61cm) 4" above knee
Guards are a little hard to come by in that size. I've tried two types of knee guards.
Leatt Airflex Pro (XXL)
This fit well and provides a basic level of protection. The pad on the knee is pretty thin, but should protect against most things other than landing on a sharp rock or something. It also has some nice pads on the top and sides of the knee. Unfortunately, one of the guards started to rip in two places after only 4 rides. So I sent it in for warranty consideration.

POC Joint VPD 2.0 (XL)
This one fit well, although a little tight. It felt much more substantial in the protection dept. The pad is like a thick hard memory foam. It covers the the knee and a good portion of the shin. It also has an extra pad on top and the main pad is fairly wide, which adds a little bit of side protection. The downsides are that it's a bit warmer than the Airflex and the material in the back tends to bunch up. I've only had one ride in these. The negatives are definitely noticeable, but I didnt really mind it after getting warmed up. I much prefer having them on. I really like how substantial the protection feels. Much better than the Leatt, while not going to the hardshell cap extreme.

I have these but listen out :)
Leatt 3DF Hybrid EXT Knee and Shin Guard (Black/White, Large/X-Large) https://www.amazon.com/dp/B00Q8MK44G/ref=cm_sw_r_cp_api_i_uSdDDbFNX6W18
I know they look bulky and more downhill oriented, but I put them on in the parking lot and I pedal up in them for hours pretty much ok. even in hotter californian days (up to 100F). Feel free to ask me what else you want to know about these guards.
Depends on what you're going for. If you want it to look like the comics, see KirbyW's comment. But if you want it to be 'realistic', I recommend something like this over a black turtleneck, with the proper blue (or red for current continuity, I guess) paint applied. For the pants, slim-fit cargos with shin guards, perhaps?
The mask is pretty easy, 'night mask', or if you want realism, maybe tactical goggles.
"Ninja" stuff might also be a nice touch (grappling hook, throwing knives etc.)
His traditional weapon is 2 eskrima sticks (buy a dowel rod from Hobby Lobby, cut it in half, paint), but collapsible batons could be an interesting alternative.
edit: added a link for goggles.
edit 2: added links for shin guards and knives.
G form is good for me on lighter riding, but full downhill, I have the TP 199 knee/shin guard .. they are pretty damn awesome !
The extra denim will add protection in a slide, but won't do much to pad you in an impact. You could pick up some cheap motocross knee pads for $20-$25 like these. I have a similar pair for dual sport riding. Another impact point is your hips and butt, not much you can do to pad those other than wearing an adult diaper, but as a bonus, you won't need to hit the rest stops as often.
I've used these for a couple years as a pitcher.
They're cheap, work great & are fairly easy to quickly put on & take off between innings.

I ride from San Pedro to Sherman Oaks 4 days a week year-round, splitting on the 110 and 405; about 38 miles each way. There are plenty of us who commute on a regular basis - I see a lot of the same guys out every day. I've never been down, and even the few close calls I've experienced really haven't been that close. It takes me around 45 - 60 minutes door-to-door. By car it's anywhere from 1:15 to 2 hours, and that doesn't include anomalous events like major accidents and lane closures.
Personally, I'd go mental if I had to do it in my car every day. Every now and then I cage it when I have to make a Costco run on the way home or whatever, and it's a miserable experience.
I ride a Ninja 1000 (less extreme ergos than ZX Ninjas). Commuting mods I've made are bar risers, a Russell seat (https://www.day-long.com/), and Givi tail trunk. As for gear, I wear kevlar lined jeans with hard shin guards underneath (http://www.amazon.com/Shift-Racing-Enforcer-Motocross-Motorcycle/dp/B005M16QY6), a textile jacket with armor, weather-appropriate gloves, and boots. I carry my work clothes with me on the bike and change in my office when I get to work, and again when I leave for the day.
If you become a regular 2-wheel commuter, I think you'll probably want a more comfortable bike. But if you can tolerate it for 45 minutes or so, you can give it a try and see if motorcycle commuting is for you. Hope to see you out there, and be safe!
these shin guards
i would get the catcher chest protector, something like this
since it's cheap investment to protect your chest and your medical bills for the just in case moment.
I use the leatt airflex. I also have the g-forms and prefer the leatt. Better fit.

Wear these under my pants, surprisingly effective for the cost.
For indoor play, I like either of these:

I'm really digging my FOX Launch Pros. I know it's on the high side of your budget, but you only get one set of knees from the manufacturer. Protect 'em!
I got started with motocross gear under a big tunic. I still fight in a slightly modified pair of these motocross knees, which are great because they actually wrap around the sides and articulate well: https://www.amazon.com/AXO-TMKP-Knee-Fluorescent-Yellow/dp/B013JSFEDM
Just regular EXT.
Edit: they might actually be the 3.0, they look exactly like these

What do you guys think of these knee pads?
they are the Evo Knee guards
for the record, i use these under the kevlar jeans
I have a pair of these and I have no complaints about them. I also wear knee pads with built in shin guards. Both of these are non-obtrusive, comfortable all day and you can't tell at all that I'm wearing them.
I do a fair amount of climbing, so I ditched my TLD Panic knee-shin guards (mainly got them to protect against pedal strikes on my shins) for some Fox Launch Enduro pads. I can easily do 2k feet of climbing and 15+ miles in the Enduro pads without issue. I don't notice them at all while riding. They don't shift or slide down my legs. They're not going to absorb as much energy or offer as much protection as the TLDs, but I believe they'll take some of the edge off a crash. Plus, the velcro from the TLDs would really bother me some days.
