I'm also on a cistern and use commercial whole-house filters in 2 stages. Stage 1 is a "pre-clean" sediment filter and stage 2 is a carbon filter that helps remove cysts and odors. The pre-filtering helps the more expensive carbon filters last longer. I also have an automatic chlorinator and contact tank for added disinfection (could also use UV) and a RO filter for drinking water. The RO is overkill since water tests fine from the tap (good idea to test periodically!), but peace of mind and all. To maintain a good flow rate, I use 2 filters in parallel for each stage (4 total). It's all easily put together with PVC. I don't have first flush and water can get pretty dirty, but after filtering it's very clean and odor free. Filters typically last about 3 months before the pressure drops too low (for 2 people, so maybe 1 month for you?) and have to be replaced. High efficiency sink and shower fittings help reduce water usage and the effects of the lower pressure. I have the pump run up to 60psi to max out the filter life.

I like to wash mine power-wash style. I turn on the high-pressure setting (here's my shower head, for reference), bend over, spread my cheeks, and blast myself in the ass. It's the most satisfying method in my opinion. Also, it relieves any itching I might have had.

I love this showerhead, it feels like a high flow but it's using very little water per minute because it has an aerator in it. Runs about 2.5 gallons per minute. Very nice soaking showerhead.

Yeah, I think this is the way to go. I use a 1.5GPM which is pretty low, but they make them down to .5GPM.

They also have adjustable ones https://www.amazon.com/Niagara-N2615-Tri-Max-Showerhead-White/dp/B008K6JW2I that can do a range.

Personally, I wouldn't want to go below 1.5GPM, but if I didn't have a choice...

Your shower head probably has a flow restrictor. You can usually unscrew the shower head and pry it out.

If it doesn't let you remove it, then you can replace it with one like this that does allow you to remove it.

I have tried a number of "great" shower heads and almost all of them are trying to do way too much and end up failing at getting you wet quickly and with enough force to feel like they are actually washing away shampoo / soap etc. Or they suck until you remove the flow restrictor and then feel you bad about how much water you're using. If you are looking for a wand style head this won't help, but the one and only shower head I can recommend is the Speakman Anystream S-2005-HB. It isn't a super environmentally friendly 1.0 gpm low flow shower head (it is 2.5 gpm) but I find that I can take dramatically shorter showers than with any other shower head and I don't have to remove the restrictor.
