Other formulations definitely have acne triggers in their formulas, so if you go this route be thorough in cross-referencing ingredients.
You can get chemical peels done on your back and shoulders, but those are better in winter when your skin won’t be exposed to the sun. I get TCA peels for $50 at a plastic surgeon’s medical spa side of the office. Salicylic acid peels could help as well.
As far as supplements: Lactoferrin specifically the Aptolactoferrin form because it binds to iron, robbing bacteria and yeast of it, accordingly inhibiting biofilm formation, growth, and also acts as an immunomodulator on human cells. Some people have luck with Zinc. There have also been studies that particular strains of probiotics that can have a positive effect on acne, and immune and inflammatory response.
Selenium, also N-Acetyl-Cysteine NAC assist in glutathione production and reducing inflammation systemically.
Responding with the cosmetic aspect of it first because if something topically is the root of what is triggering or worsening the acne, supplements can be a steep uphill battle over what possibly could be just a mild one, if any at all.

Personally, I take them on my 48/72hr fasts. I'm 40/F and I just did an 80-ish day fast. Vitamins/minerals/electrolytes only for the first 20 days, and then just enough dietary fat to stave off the hunger pangs (about 200 calories twice a day).
The Minnesota Starvation Experiment revealed that there's a rate limit to how quickly body fat can be converted to calories, and it's a function of how many pounds of body fat you still have. So if you've been on a total fast and suddenly get blindsided by an all-consuming hunger pang that won't go away, that may be your problem. Both carbs and protein raise your insulin, which inhibits fat-burning, so you'll still want to keep your calories during the social events to a minimum, even if they're keto calories. It won't be difficult; your stomach will probably be the size of a marble. LOL.
The sex difference may be a difference in the muscle tone of the heart, since elevated adrenaline after day 5 makes your heart work harder, so if you're not in the best cardiovascular shape, you'll feel fatigued all the time, similar to heat exhaustion, until your heart adapts. I started feeling amazing on day 20 or so, just before I had to start supplementing dietary fat.
Guys... Stop blaming our periods for everything! LOL. I had three totally normal periods during the fast. Zero changes in my menstrual cycle the whole time.

Think about it: if you have one supplement that says 100 mg magnesium xyz, and another that says 100 mg elemental magnesium (from 2000 mg magnesium xyz), and both pills are roughly the same size, does that add up? It doesn't to me. And here's another thing: I didn't start having the vivid dreams and waking up refreshed until I started taking the magnesium laxative at 1 fl oz. 1 fl oz is 1,745 mg of magnesium cirate, with 282 of it being elemental magnesium. When I was taking it in tablet form, I was taking 400 mg of magnesium citrate, but I never experienced any differences in my dreams or felt more refreshed waking up.
Since I haven't heard from you, I thought I'd drop some info on you. By the way, I don't mean to be pushy, but I have some experience here and had to research my way out of a lot of it. Also, I hate seeing people suffer when they might not have to.
Anyway, when I had gut issues, I was swinging between constipation and the runs. I was always hungry and only felt good for the first hour after a meal. After that, I was drowsy, lethargic and irritable. Consequently, I ate too much just to get through the day. I got testing for food intolerances/allergies. I had through-the-roof antibodies present for eggs, milk and almonds. It wasn't causing a face-swelling, throat-closing reaction - that would be an allergy, which is life-threatening. But my immune system was reacting enough to cause the symptoms.
Of course, I cut those foods out immediately, which helped, but it wasn't enough. That was when my doctor started calling around for help. What he came up with was a product called Inflammacore. It's a powder you mix with water that has a bunch of soothing and anti-inflammatory food and plant products. It also has a good amount of caffeic acid phenethyl ester, which is a pretty expensive ingredient. (look it up, it's interesting) In my own research, I found out about pre/probiotics. Long story short, probiotics are microorganisms that colonize the gut and regulate the processes down there. Prebiotics are food for the probiotics, because they have to make it through the stomach and small intestine, which takes a few hours. The food helps keep the organisms alive on their perilous journey.
For me, after about a week of taking 1 scoop of the powder with pre/probiotics in the morning and another scoop by itself at night, my appetite decreased and my moods stabilized somewhat. I did this for 3 months and got better and better. I still had to go on antidepressants later, but it was a big difference.
When you say you have recurrent diarrhea, it sounds to me like your guts are inflamed and angry about something, whether it's stress, or a bug, or some food that doesn't agree with you. I would guess that if you tackle that first, you might get a leg up on the depression. Did you know that 80% of your serotonin is produced in the gut? That's not some new age bullshit, you can look it up.
I'm not going to act like I know the first thing about narcolepsy, it's completely outside my experience. I was tired all the time, but I'm sure there's a difference.
