If TSH is high, that is like your pituitary glad screaming at your unresponsive thyroid, so no surprise that your thyroid hormone levels were also low. That usually indicates that you need to increase your dose.
I have Hashi's and I take 146 ish mg of NatureThroid along with Hyperbiotics PRO-15 Advanced probiotics, B-complex with coenzymes, Iron, Magnesium, Omega 3-6-9, Selenium and D3-5000IU. I have tried to reduce the number of supplements I take but they all help. I sometimes take Zinc as well. I quit coffee and take caffeine pills instead and I am (not super strictly, but working on it) gluten and soy free.
The first time I took Nature Throid after switching from generic levothyroxine all of my symptoms were about 80% GONE. We increased my dose from 1/4 grain to the current 2+1/4 grain over the course of maybe 6 months.
What really fixed things for me was going gluten and soy free and adding magnesium and selenium. The selenium reduced my antibodies from nearly 500 to 100 in six months. That alone made a huge difference. And going gluten/soy free allowed me to lose 20 lbs in a month and then maintain it pretty easily just logging my calories.
Occasionally, due to poor planning, I eat regular bread or some pizza and I am sapped of energy and achy for a week afterwards. I'm working on getting into the habit of always having gluten free options on hand. But generally speaking, I feel better than I've felt my entire life.

Last reply before I need to get out of the house. Yes, fighting hunger can be tough, especially in the last hour or two of the 16-hour fast. I fight it by drinking water and coffee (not to excess, just a cup when hunger begins), and by staying busy with work to keep my mind off it. [Edit: R-Lipoic acid also has proven appetite-suppressant, weight loss effects.] It gets easier with time, as your body learns to accept a certain level of hunger, which sometimes even passes on its own. That, I believe, is a function of the body's improved handling of insulin levels. Just keep in mind that the human body didn't evolve to be constantly fed. For most of our evolutionary development, we experienced frequent but temporary levels of hunger as we hunted and gathered, before agriculture gave us a more steady supply of food, so remember that enduring a bit of hunger is actually much more healthy and natural than feeding the sensation the moment it pops up. After just a few weeks of intermittent fasting, contrary to myth that "breakfast is the most important meal," I felt noticeable improvements in energy and mental clarity throughout the day.
For my eating window, I don't bother trying to eat a commensurate amount of calories. I just eat normal-sized meals, without eating excess carbs (I'm not fully keto) in order to avoid blood sugar spikes and crashes. I have lunch at the end of my 16-hour window, then a protein bar or some trail mix for a light snack mid-afternoon, then a normal, healthy dinner. When I have lunch the next day depends on when I eat dinner (+16 hours, of course). I consume absolutely no food or sugar during the fast period (drink coffee black), as this will cancel out the benefits of the fast.

Bacopa will be replacing Ginko and Alpha GPC will replace the lecithin. Any other suggestions are welcome, the stack in its current form is a bit unmanageable however it helps to empty the capsules into the protein smoothie in the morning and into a blueberry keifr milk smoothie in the afternoon or early evening.
I'm no expert on the matter, but after spending years researching and going to multiple doctors, I finally came across something called small-fiber neuropathy. Look up the symptoms, see if they match up with what you're dealing with, since symptoms can range so much for people. Fibro is confused with small fiber neuropathy a LOT (which honestly I feel like fibro is just a blanket diagnosis when a real diagnosis can't be determined, but that's just my opinion). There is actually a test for small fiber neuropathy where they do a biopsy. May be worth looking into.

I can NOT swear by it enough. My nerve pain was almost 100% gone after a week of taking one pill twice a day with meals (600 mg total). When I finished the bottle I tried another brand and it was NOT effective, the nerve pain came back. Got another bottle of the BRI brand, and again after just 2 days I can feel it decreasing. I stopped taking 2/3 of the prescriptions I was on.
I wish I could share this with EVERYONE because most people I've talked to have never heard of this. And like most people, I had been placed on every drug imaginable with little to no improvement except just being a numb zombie, who likes to live like that!?

I am buying nerve health supplements because I am recovering from an acoustic neuroma - (and with vitamins many doctors think they do nothing - so keep that in mind) Nervestra Nerve Health Support Supplement, Alpha Lipoic Acid Vegetable Caps, Methylcobalamin (Methyl B12), 1000 mcg, Lozenges and Centrum but I am not taking anything for the ear for hearing health maintenance.
Anyone want to recommend any vitamin/herbal supplements for hearing heath maintenance?
While I am aware of medical literature that says vitamin supplements do nothing - it never made sense to me why vitamins absorbed in foods are good but via pills is bad (if the pills were of good quality and contained the ingredients they contained).

If that's still too complicated, then just down a mix of maltodextrin and wazy maize. Also good to include some BCAAs, creatine, and antioxidants (e.g. ALA) into the mix as well.
Yeah they're a little lower than what's recommended but they'll work. They've got the vitamins but are lacking magnesium, which helps with jaw-clenching. And TUMS taken when you drop will extend the roll.

Clen definitely can assist in weight loss if it's used correctly. There are significant risks: you must drink at least 120-160oz of water every day because... you must be ok with sweating A LOT on days that you take it because it lasts a long time and it's not well suited for gym in the morning and then going to the office. You can only cycle it for 2 weeks at a time, and for a total of 12 weeks per calendar year. It will most likely make you very vascular (an enviable aesthetic for some or dysphoric for others) and it's pretty hard on the cardio system. If your AST and ALT blood levels are in the median reference range or lower then you should be ok to take Clen as long as you take cycle support + 2-5g of taurine every day.

What is your stance on caffeine as far as disrupting / interfering with estrogen uptake and feminization?

Here's the thing about those research studies... they're all done on cis-women who are not injecting estrogen or taking estrogen pills at the extent that trans-women do. If there are issues with caffeine (first off you have to be drinking a lot of caffeine for this to be an issue in the first place, and that's not generally healthy anyway + there are better/healthier stimulants than caffeine) then we, as trans-women, can simply take more estrogen and correct the imbalance. Until more research has been done, and focused on cis-women who have had a single/double hysterectomy (this is the closest demographic we can get to in the medical community if they aren't studying trans-women specifically) then the issues with caffeine can be disregarded since we do not fit into the study or control groups (no menstrual cycle).
Benefits? I went from almost falling asleep during the day to bursting out of bed in the morning to still having energy into the evening. Although ZMA (Zinc, Magnesium, B6) has improved my sleep quality 1000% which helps a lot, but I just started taking it regularly recently.

It is part of the subscribe and save program which is awesome because I note how often I need them shipped (every 3 months, 4 months, etc.) and it ships them then to me automatically.
I have read that Na-R-Ala was better because it had a longer half life, so you could take it every 2 hours instead of every hour. I couldn't find any, but I got some Ala time release which releases 600 mg over time instead of taking 150 mgs 4 times during my roll. It's better than nothing I guess. And I will also take vitamin C tablets, forgot to mention it in the post.
You may want to consider supplementing Na-R-ALA alongside the ALCAR because it can be oxidative at higher doses although I've read that under 2g per day should be okay. I've seen ratio suggestions of 2:1 or 5:2 for ALCAR:ALA.
Regardless, I err on the side of caution, plus ALA is a great supplement that synergizes with ALCAR. I take one Genceutic Naturals R-Lipoic Acid 300 Mg, 60-Count with 500-1000mg ALCAR.
I would also suggest that you take fish oil with CDP-Choline because they synergize as well.

ALA in it's alpha form isn't the best product to take. You end up pissing out most of it. It doesn't absorb well at all. Ever notice your urine stinks when you take it? R- Lipoic Acid is absorbed much better.
R-Lipoic Acid This is the one I take. I started taking it for the very reason you claimed, it helps when you overdue a carb here or there and with me being hypoglycemic and occasionally having bouts of reactive hypoglycemia (esp when I eat to much protein) it helps keep that under control.
