I also did not want to put my girl on medication unless it was my last resort so here is what I did and thankfully it worked wonders!

I also have been feeding her a raw diet for a few years now - when I first adopted her, her skin was horrible and she had ear infections every month. Switching to raw food improved her coat, stopped her ear infections and helped her overall health. I also only feed her natural treats every once and awhile, nothing packaged, such as apples, sweet potatoes and sardines.

Edit 2: just remembered, my dog was initially reluctant to let me put it on, so I gave her a treat and praised her after the first few times. She soon associates the application of the lotion with getting a treat and now lets me do it with ease.

There are fewer ingredients in this compared to Nizoral, and most of the ingredients I at least somewhat recognize. I do contact therapy - leave on my face for 5-10 minutes then rinse off. Only been using this for about 5 days, but so far so good. Good luck!
My shih tsu, Frodo, had the same problem which my vet said was due to his seasonal allergies (itching, breaking the skin causing tiny sores which would then cause him to itch more and sores would get infected... a vicious cycle). I would get him a shot to control the itching twice a year and have temporary relief from the smell until the perpetual itching would start again.
It was my sister (who is a veterinarian) who was visiting one year who finally gave me the solution that worked. Frodo and our family suffered with his itching and his smell for over 8 years. He was not allowed on furniture or beds bc of the smell. His last 9 years were smell free & itch free, hallelujah! Her recommendation worked so well that he never needed to get the shots anymore.
My sister's recommendations:

It's the only Ketoconazole product I could find that was not a shampoo (I don't understand why). I like this for a leave in, but I don't know the effect of that antiseptic. It's safe, but it's not made for long term used I'd imagine so use at your own risk
My dog has terrible allergies as well; We had him on Apoquel for awhile but the side effects scared me. Through a lot of trial and error, we finally have a natural remedy that works for him. He's on a half kibble/half raw diet, gets 1 TBSP of Apple Cider Vinegar each day and 1 TBSP of Coconut Oil. I also give him these allergy treats twice per day, and invested in some grooming wipes so he can get regular wipe-downs between baths to get any allergens off of his skin and paws. My puppy had the exact same thing on his armpit at 10 weeks old. They look gross like acne and have scabs from the dried puss. Like the other poster said, I think it's puppy pyoderma which is common for young puppies, usually doesn't bother them, and it goes away on its own. I bought some otc pet topical that contains Chlorhexidine Gluconate and sprayed it liberally 2 times every day on the area. I could tell it was working. He is 6 months now and they're completely gone.
My GSD had food allergies, figured out she could not have beef products of any kind, in food, or like a bully stick/beef treats. Switching her over to Origen 6 fish made a huge difference. Also, I applied 2 sprays to her paws (she would lick her paws and belly during an allergic reaction). Vetericyn and another spray that had tea tree oil. my dog did not like to lick tea tree oil. However do not use tea tree oil full strength on dogs. And never use on cats.

You need to use actual Medicated shampoo and sprays. An oatmeal shampoo isn't going to do anything for allergies. You might need to figure out which one works for your dog.
You need to bathe the dog often to remove whatever allergens are on him. Might even need to wash his feet every time he walks through any grass or plants.

TIL Amazon sells Johnson's Bitch Spray for sale in the UK. It "disguises natural oestrus odour of bitch in season" and "protects bitches from unwanted attention from dogs whilst in season." Sorry reddit, can't be used for cats.
